Fire reports are considered a matter of public record and are available upon request.


Investigation reports will automatically be included with the report if the fire was ruled accidental. If the fire was ruled undetermined or incendiary, text in the report will indicate that the report is not considered public record. To obtain a copy of this report, please subpoena the Custodian of Records for the Clark County Fire Department.


The emergency medical service reports and the information they contain is protected
by law and will be released only with proper authorization. See the details below.
PATIENT
  • The patient must pick up the report in person.
  • Valid photo identification is required when picking up the report.
PARENT OR GUARDIAN OF A MINOR PATIENT
  • Minor is a person under 18 years of age.
  • The report must be picked up in person.
  • When picking up the report, the parent or guardian must present his/her valid
    photo identification and a birth certificate of the minor patient.
ATTORNEY OR INSURANCE COMPANY
  • Original notarized medical release form is required if someone other than the patient
    or parent/guardian of a minor patient is requesting the report.  This form MUST be mailed.  Do not fax.
  • If the patient is unable to sign a medical release form, the requestor must have a subpoena in order for the report to be released.
  • The report can be faxed or mailed to the requestor upon proper documentation.
DECEASED INDIVIDUAL
  • A report for a deceased individual may be released to the personal representative
    of the estate upon presentation of a death certificate and a court order showing the appointment of the personal representative.

It may take up to 30 days to process your request. Submitting duplicate requests will not expedite the process.
